PACE is designed to offer health care services to seniors in the Community. We work as their Health Insurance as well as their care provider. We coordinate and supply services with the goal of keeping individuals in the community and out of Long-Term Care. Some of those services are:

Prescription Drug Coverage

24/7 emergency on-call access to a staff member

In home Assistance (personal care, housekeeping, meal prep)

In Home Nursing Services

Medical Transportation

Social/Medical Adult Day Center Services

Physical/Occupational Therapy

Nutrition Evaluations

Medical Supplies/DME (Walkers, Wheelchairs, Hospital Beds, Stair Lifts, etc...)

In-Patient Hospital Care

Specialty Care (Cardiology, Ophthalmology, Podiatry, etc...)
